We now have elctronic check-in through PHREESIA. You will be sent a link by text or email to complete your on-line registration and check-in process up to 3 days prior to your appointment. Please make sure to complete so that it is a smoother transition.
If you pay your copay or deductible prior to the visit day you will not be charged until you are actually checked in.
This allows the payment to cancel if you are unable to make your appointment.

Take pictures of your ID & inurance card and upload. Your child's information will be required to be verified at every visit. You may need to re-schedule your apt if not completed.
