The Providers at CCC care deeply about the health and well being of all of our patients. We also believe that vaccines are the best way we know to prevent disease. In order to continue to ensure the health of all of our patient population at CCC we have implemented the following changes effective April 1, 2019:

We will no longer be able to take on new patients from unestablished families who have chosen not to vaccinate.

If you are a new family to CCC and on an alternative schedule, we will ask that your child be fully immunized by the age of Two (2).

If you are an established family at CCC and choose not to vaccinate as recommended by the American Academy of Pediatrics and Center for Disease Control we will require you to sign a refusal to vaccinate, acknowledging the risks and that you have been counseled about them.

Any of the above guidelines can be discussed at your well check with your provider.

A Biological Parent or Legal Guardian MUST accompany child at first visit. We ask that you bring completed New Patient paperwork, valid picture ID, Insurance Card and your child's immunization record.

What's New?

WE NOW CHECK IN WITH PHREESIA - You will be sent a text or email prior to your appointment. Please go through every prompt and asnwer all questions. Take pictures of your ID & inurance card and upload. Your child's information will be required to be verified at every visit. You may need to re-schedule your apt if not completed.
