Referrals

If you need your child to be referred to a specialist, please call our office and leave the following information:

1) Choose a provider and location you would like your child seen at.

2) Please call the specialist to ensure they take your insurance, make the appointment with chosen specialist. If they do not schedule until records are sent please call the referral line below.

3) Call our office at 602-978-2500 and choose option 5, the referral option. When it prompts you to leave a voicemail please leave a message with: doctors name, phone & fax number, type of specialist, time and date of appointment, or if records are needed and a your call back number. 

Some referrals may take up to 7 days to process so call as soon as possible. 

If you have Health Choice prior authorizations may take 4 – 6 weeks to obtain.

All AHCCCS plans require a referral and prior authorization for many of the specialists. Please remember that your child needs to be current on their well checks in order for us to process any referrals. Well checks are scheduled at 2mo, 4mo, 6mo, 9mo, 1 yr, 15 mo or 18mo, 2 yr and once a year after that.

If your child has been seen at the Emergency Room or Urgent Care for an injury, please make sure to request those records, including labs/x-rays be sent to us prior to calling as they will be needed to process a referral.
