New Patient Forms

Please check to see if we are contracted with your insurance and have your insurance information available when calling.  Once you are made an appointment your new patient forms will be text or emailed to you through Phreesia.  Biological parent/Legal Guardian must be at the first visit and sign all necessary documents.  Legal Guardians or parents that have a Notice to Provider are required to bring this to their first appointment.

If your child is a newborn, please call your insurance and add them immediately.  Please remember to bring your insurance card, photo ID at every visit.  These can also be added to the Phreesia check-in by taking a photo with your cell phone.  Please arrive 15 minutes early to complete any forms that were not received.

Remember documents/Phreesia can only be completed by parent or Legal Guardian.  Parents will have the opportunity to allow family members/friends to bring their child in for future appointments by completing the Permission to Treat page. 

Please request your child's records from their previous office to be faxed or emailed to our office.  We cannot accept CD copies of charts.  You will be required to print them & bring them in.

WE NOW CHECK IN WITH PHREESIA - You will be sent a text or email prior to your appointment. Please go through every prompt and asnwer all questions. Take pictures of your ID & inurance card and upload. Your child's information will be required to be verified at every visit. You may need to re-schedule your apt if not completed.
